knocking and vibrating from underneath the car

have a knocking and vibrating sound from underneath the car when im driving on the motorway. it seems like it happens when i put my foot down then take it off again. could it be a build up of pressure in the exhaust or a knackered cat? it makes the whole car shake and vibrate when it happens.
